BBC Verify has identified 254 incidents attributed to the Israeli military in Gaza, Lebanon, the West Bank, and Israel with no public investigation outcome.

Using AI tools to examine more than 60,000 news reports, BBC Verify found 300 cases the IDF said it was investigating, with outcomes published for only 28.

The incidents between October 2023 and August 2026 include 158 in Gaza, 48 in the West Bank, 32 in Lebanon, and 16 in Israel or Israeli detention.

The IDF said it publishes investigation results "when possible," subject to legal, operational, privacy, and information-security considerations.

Data obtained by Israeli rights group Yesh Din shows 1,456 Gaza-related incidents were referred to Israel's fact-finding mechanism between October 2023 and August 2024.
