Tennessee's Corrections commissioner is stepping down after a failed lethal injection left death row inmate Christa Pike unconscious on a ventilator.
- Christa Pike, 50, remains hospitalized on a ventilator after two doses of pentobarbital failed to kill her on September 30.
- Gov. Bill Lee announced Commissioner Frank Strada's resignation, ordered an independent review led by former US Attorney Ed Stanton, and halted executions this year.
- Pike's attorneys allege improperly placed IV lines allowed the pentobarbital to enter tissue rather than her bloodstream, severely injuring both arms.
- Strada, who oversaw three execution mishaps in Arizona and a failed Tennessee execution in May, said stepping aside was "in the best interest of Tennesseans."
